- Health Assessment Record form, required for participation
- Medication Administration, if applicable
- Individual Plan of Care (IEP), if applicable
*The Health Assessment Record and other applicable forms must be mailed or dropped off at the Garde Box Office no later than July 1. No emailed or digital copies will be accepted.
Attendance and active participation are required for all program days, rehearsals, and both performances.
Scholarship Funding for families based on financial need may be available through the Garde's P:ACE (Partners: Arts, Community, Education) program. To apply for scholarship funding, please call the Garde Box Office for more assistance, (860) 444-7373 ext. 1. (do not register online)
Refund Policy: 100% refund 45 days or more before the first day of class, 75% between 25-44 days prior to the first day of class, 50% between 25 days to 7 days before the first day of class. No refunds less than a week prior to the first day of class.
